Project HOPE - HABITS for Life
The "HABITS for Life" initiative is a multi-faceted program designed to address health disparities in New Mexico. The purpose of the program is to promote healthy habits by offering comprehensive health education and awareness campaigns, increasing access to health screenings, delivering train-the-trainer programs that target multidisciplinary health workers, and increasing access to health care via telehealth. Program components include:
1. Healthy Habits for a Healthy Weight. We create school- and community-based awareness with active learning modules and community projects in an environment geared for success.
3. Training and mentoring multidisciplinary health professionals. We boost clinical and educational skills to strengthen patient self-care, growing a network of patients who can motivate others.
4. Serving underserved populations. We improve access to primary and specialty care health services via state-of-the-art telehealth technology on the HOPE mobile.
